If you are new and would like to begin our intake process, please call or message us at 334-690-4673 or at [email protected]. We will need your full name, date of birth, and a good email address in order to create you an account on our client portal. Once we’ve entered your information you will receive a link by email to create an account with your username and password. We recommend you save this information and bookmark the website in your browser, as you’ll continually use this site to complete paperwork, schedule appointments, and update payment information. Once your consent and intake forms are complete, our staff will review your documents and get back with you about an intake appointment.
